My account maxed out on storage, so the form was disabled. I deleted all the data so we have space in our account again, the settings on the form say it is enabled but through the web site I get the message "Form Disabled" when I try to submit the form.

Your form is up and running perfectly now. Our servers are experiencing some mild hiccups today due to release of dropbox integration. That could be the reason why your forms took some time to get enabled. Have a nice day!
