How to fix incorrect email notification template

    I created a catering request form and 3 of the questions show

    "type a question" in the description field after the form is submitted.  I deleted them and redid it twice but this did not help.  Not sure why it is doing this.

    This is usually happening when the form email template is outdated. The solution is to re-create the email notification:

    1) Delete current email notification

    2) Add a new email notification
