Hello after inspecting your form and the screenshot you've provided I think that the best solution to solve this problem would be for you to delete your current email notification and make a new one. This will allow the Email Notification Template to be rebuilt based on the fields that you have available on your form right now so that they match.
If you don't feel comfortable with doing this then what you will need to do is go into your Email Notification for the form and then manually modify it along with removing any questions and field answer tags that do not apply then add them in so that they coincide.
