Questions about sumitted form from a client or patient.

    Hi,  I am using the Medical Form template and I have a few questions:

    1) When the client/patient has filled this medical form out online and submit the response, what does the response look like?  Would it be exactly like the original questionnaire with the answers or is there some consolidating, reformatting of these answers to make them more useful for the doctor to digest?  Is there a way I can see what a response look like after the questionnaire has been submitted?  We suspect that it will look exactly like the input format but of course with the answers filled out.  Is that correct?  So if our template has 20 pages, the responses will be 20 pages?  No way to consolidate answers or reformat them to make them more digestible.  For example, only show the answer that is chosen rather than all the options.  Please let us know.  Thanks.

    1). what does the response look like? — What they see in this form http://www.jotform.us/form/90626629486166 is what they will fill in.

    2). Is there a way I can see what a response look like after the questionnaire has been submitted? — That can be viewed in your email notification. Please check this guide https://www.jotform.com/help/25-Setting-up-Email-Notifications 

    3). So if our template has 20 pages, the responses will be 20 pages? — Are you referring to PDF template? If not, what you see in your form is what your customers will see online.
