Why does my survey switch the location of question and answers when I download the PDF?

    I want the applicants name to show up first but instead a question from page 4 shows up first. Also why doesn't the PDF print the logo and the information below the Standard Application title? I want the application to be downloaded into a PDF and look the same way.

    I am sorry for the trouble caused to you. I did try to check your form's submission PDF template and it shows the message that you have changed the form. Please check the screenshot below:

    Please note that changes made to  form will also display in submission PDF as well. I would suggest you to please go to your form's submission template in PDF editor and add a new PDF document where you can rearrange the form fields and also add logo as well.
