    I guess you're asking how to build a form where respondents have the option to enter one set of information for a dog (or more dogs if applicable). You can achieve this by using SHOW / HIDE CONDITIONS. The idea is to ask them how many dogs they want to register and depending on their answer, you will conditionally display more fields (e.g. if you they want to register 3 dogs, you'll show 3 sets of fields for 3 dogs too).
