Uploaded form restricted to one entry

When the pet application form is published to my website and I try to test is, I am informed that only one entry is allowed.

How can I fix this to multiple entries?

This is usually related to Unique Submission feature being enabled on the form.

To allow multiple entry again, set the option to No Check

Please test submit the form again. It should now allow multiple submissions.
