Hi, I have a form (preview: http://form.jotform.us/form/30345617953154) in which I would like to add a page break just before the final section of my survey (between pages 2 & 3). When I put the page break in, my form cuts off several lines above where it should, and the previous and next buttons are not displayed. If I take the page break out, the survey displays in its entirety.
Here is a preview of the live version of the survey: http://form.jotform.us/form/30079133391147. You can see at the bottom of the second page that the page break has not been inserted and the form displays in full.
Can you please help me figure out why the page break is causing the form to end before the actual break?

I have fixed it on this clone version of your form by setting up the labels of the checkbox fields (just before the upload button) to TOP and by putting at least a blank space on the field labels.
