Feedback form with unique coupon code upon completion

Hi there, 

I need to create a customer feedback survey which allows recievers to complete only once, and once completion of the form, they are to recieve a coupon code. 

The coupon code needs to be also shared with me for reference. Can I know how I can go about making this happen?

Yes it is possible to achieve your requirement and here are the steps you need to follow:

#1. To generate code for every user, please add "Unique ID" widget in form and customize it based on your requirement. You may like to take a look at the following guide which should help you:  https://www.jotform.com/help/79-How-to-Add-Custom-Unique-IDs-to-your-Form-Submissions

#2. You can add notification email alert in form to receive the notification email including the unique id. The following guide should help you how to add notification email alert in form: https://www.jotform.com/help/25-Setting-up-Email-Notifications

You can also go to your form's submission page to view submissions including unique ID which was generated per user. The following guide should help you how to view submissions:  https://www.jotform.com/help/269-How-to-View-Form-Submissions 

#3. Please note that there are two possible options to prevent users from submitting forms again:

a. Unique Submission: You can take advantage of our unique submission feature to block multiple submissions from one system of one IP address by setting the "Check Cookies Only (Loose check)" or "Check Cookies & IP address (Strict Check)" respectively. b. Unique Question: You can also use our unique question feature to prevent from submitting duplicate submissions. Example: You can set one of your forms question (like cell number or email address) to be a unique question. This will not allow a user to use the same cell number or email address to submit your form.
