Can I create a fillable form that looks just like a document that currently exists?

I've been tasked to see if JotForm can create a fillable form that looks just like a document that currently exists.  Basically have a form that looks a certain way be fillable on JotForm...

Is your existing document a fillable PDF form? If it is, you may import it as shown on this guide: https://www.jotform.com/help/548-How-Can-I-Import-My-Own-PDF-to-JotForm
