Required fields are empty in the submission

    Hello- I have a form that requires people to give me their name and email address.  But I got a notice in my inbox that someone had filled out the form and when I went to look at it- it did not have their name or email address

    I have included the URL for the link

    and the screen shot of what I actually got back- I could not fit the whole thing into one screen shot but the info I did get started with the phone number and all the info from there until the end of the form was filled in.

    I am most concerned about the missing name and email address.  This person is new to me- I can see that because that part of the form is filled in but I have no idea who it is.

    I know I can't fix it for this form but I want to make sure it does not happen again.  I do have the form set up so the name and email address is required.

    I have tested a cloned version of your form and it seems that the inbox shows empty fields.

    However, the number of adult field is not required, therefore if the user skips this field,it will not trigger to show/hide condition with the name and email field.

    You can make the Number of Adult field to be required so that your condition would be triggered.

    Yes- this does help- I made the number of adults required now so hopefully
    that will take care of this issue.
