How to make a waiver form with next and back button?

I am wanting to know the best way to create a waiver where people can add children and go to CONTINUE or BACK tabs.

You can use the page break to have a back and next button to your form. You can find it on the "Add form elements" under the BASIC tab.


And for [Add another child] similar to the webpage you gave us, you can use the configurable list widget or the infinite list widget.
