I need to send notification (results) to an email address the guest pro

I need to send notification (results) to an email address the guest provided on the form. How do I do that?

In order to send email to form respondent, please set up an Autoresponder email template. Here is how: Setting-up-an-Autoresponder-Email

Thank you but I dont' think this works for what I need to do.  The results need to go to an address provided by the guest completing the form from a field on the form.  it will not go to the same email address every time.  I don't see where I place the field name for the "send to".

This is exactly the case for Autoresponder email. When setting it up, you can choose field on 'Recipients' tab, here is a screenshot:
