I need to update a form

I am trying to update the form to show the new price and date per the request of the college. king University has ask me to update the site.
The coach who started this project has move on to another coaching job. And I need to update the form. The coach there is Michael Phelps.
He is the college rep who ask me to change the website. You can confirm the request with him.

You can log in with the username or email address. If you don't know the password, but you have access to that email address, you can reset the password.
