Forms created in Jotform and Login issues

    I created several at least 3 forms in JotForm.  When I attempt to use them I receive an error message stating the form(s) are disabled.  Then go back into the program it will show the form is there and enabled when I copy the link it doesn't work.  Then when I go back again it will show I haven't created any forms.  

    Secondly, when I  attempt to log in I get an error message login failed account suspended.

    These forms were found on your account as suspicious of phishing activity because you're collecting social security numbers and flagged as credit application scam.

    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    If your purpose is not to collect social security numbers from the general public, but from your own customers, and if the nature of your business is listed above, then you need to comply with two things.

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security cards, driver's licenses, credit/debit cards and statements, bank statements, voided cheques, birth/marriage certificates, visas and photo IDs.

    As for the credit application scam, we understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

    In order to reactivate your account, we will have to delete the offending form.

    Or, kindly provide us the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to with Thread #1756756 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Thanks for your quick response. I am a Credit Repair Specialist.  The forms were created so that I can properly and accurately perform Credit Enhancement Services and provide other financial solutions to my clients. Please allow me to modify the forms for review.
