Form over quota

Jotform on our www.yorkfitness.co.uk website is showing "Form over Quota". 

Looking at the usage stats we should not be. 

Please can you help resolve as soon as possible. 

I checked on your website and it shows the form is active and available.

I did reviewed your account usage limits and none appears to be over the limit.
