Export the Excel of the Form as there is an issue in Login

On checking, I am not able to replicate any issue with your account. We were able to create a Demo Form.

The account to which these Forms belong is sasithorn_SRH. You may reset the password of this account if you do not remember the password. We cannot extract the data from the Forms of another account. The request for an operation on the Forms should come from the Form owner.
