    I need to know why my account was suspended again. I thought everything was fixed the last time. What is going on now?

    These forms were found on your account as suspicious of phishing activity because you're collecting passport/ID numbers.

    Online Form Builders, including JotForm, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    If your purpose is not to collect these data from the general public, but from your own customers, then you need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security cards, driver's licenses, credit/debit cards and statements, bank statements, voided cheques, birth/marriage certificates, visas and photo IDs.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending fields mentioned above.

    That was removed the last time. The cloned was not in use but a Template. I
    dont understand how its still showing up.

    As I checked, my colleague removed the offending fields on your form "Bobby Bday". However, upon checking, there are more forms like this that collects passport and ID numbers without the mandatory widgets.

    Also, our phishing detector still detects violations even if the form is already disabled or deleted. It is a must to remove the offending field on your form or purge the form from your trash folder after deletion. This is to avoid getting flagged again by our system that can lead to this kind of inconvenience.

    I've removed the offending fields on your forms and lifted the suspension on your account. Try logging in now and let us know if you need further assistance.

    Oh ok. So even though I deleted they were still there? I saved her email
    saying what to do with the terms and conditions. Thank you

    Yes, deleted forms goes to the trash folder. This gives you a chance to restore your form from there if you happen to accidentally delete it. That's also why there is a purge option there to completely remove a form from your account.
