I need to design a form that allows users to create questions with multiple choice answers. The number of questions will be dynamic so they need to add and remove questions at will.

I need a widget that's a combination of Dynamic Textbox and User Contributed Checkbox. These two would work well but they need to be linked together on the same "row" like a spreadsheet.

    Is this possible?

Unfortunately, it is not possible to have a dynamic questions.

However, you may setup your form to have conditional questions.

    You may check this guide:https://www.jotform.com/help/57-Smart-Forms-Using-Conditional-Logic

I'm not sure we have a widget that would fit your requirements.

May we know how you would like your form to work?

You may try the input table .
