I need to design a form that allows users to create questions with multiple choice answers. The number of questions will be dynamic so they need to add and remove questions at will.
I need a widget that's a combination of Dynamic Textbox and User Contributed Checkbox. These two would work well but they need to be linked together on the same "row" like a spreadsheet.
Is this possible?

Unfortunately, it is not possible to have a dynamic questions.
However,you may setup your form to have conditional questions.
