The form looks different when published then it does when editing it as the box labels at the bottom are longer on edit

Additionally, now the form looks different when published then it does when editing it as the box labels at the bottom are longer on edit (the way it should look) vs. formatted into short columns on the published version.

I am reviewing your form and it seems to look the same for me in edit mode and via direct link. Could you please provide us the screenshot of where do you see a problem so we checked closer?

I was able to resolve that issue, but the form is unuseable!
When I try to use the form I receive this error:
