Can I attach a document

I have created a registration form for kids camp.  A required single page document to be downloaded and printable from our registration form for the parents to take to their child's physician, have the Dr. fill out and send to camp administrators.  Is this possible?

I'm afraid that you cannot make it required to be downloaded from the form, we don't have such feature in the form, you can attach the file to the form to be downloaded via Text field:
