I submitted a bunch of test forms but rather than going to the email address attached to the form (mine) they showed up as submissions on your website. Is there a way to direct submissions to an email address?

Hi,
Yes, you can set a notification email for each form submission.
Firstly, you have to set a Notification Email for the form.
You'll find the Notification Email feature under the settings menu for each form:
When you select Notification Email, you will see 3 tabs, one of those will be Recipients.
You click the Recipient tab, and put the recipient emails in the input field.
And now when someone fills your form, you will receive an email.

Yes, I can confirm that you have set up the email correctly. If you are not getting the emails for your tests, you should check your spam settings and make sure to whitelist Jotform. You can find our list of email addresses and domains in the link below:

Is it possible to confirm these emails are actually being sent when I click on submit on the attached form? I checked and do not see that our email server is blocking them.

I checked your email logs and there's indeed no email sent from the form, but you have 9 submissions there. Did you just create the current email templates? I rechecked your settings and, they're all correct. I don't find any reason why you would not receive the form emails.
Anyways, I sent two tests in the form, and they are sent based on our logs. The form should work now, but if the issue persists, please update us here so that we can troubleshoot further.

I'm sorry, but I'm baffled. Are you submitting using the direct link of the form which is https://form.jotform.com/90573113442147 or through PDF? Are you submitting on a specific device? A mobile or a PC? My latest test before this reply is through the PDF form - submitted using Adobe Acrobat app installed on my Windows PC.
I noticed that in your test submissions, you were able to submit Off and Off and so on in the Delivery Details (Check all Applicable) field, but you do not have these options in this field. I think that's the reason why no emails are sent because these options do not match.

I was submitting this PDF from my virtual desktop. I have Foxit pdf.
When you say Off and Off, what are you referring to exactly? That sounds like an interesting lead.
