PDF editor: Fields are placed randomly in the PDF document

Hello there, 

When i am downloading a copy of a submitted form (PDF)--  The phones number are not being displayed under the individual's name. They are being placed randomly throughout the downloaded form. Can this issue be resolved?

Could you let us know the form ID/URL that you are experiencing the issue so that we can guide you in the right direction? Please try creating a new PDF document in the PDF editor and see if that helps.
