Login Issue

Upon review of your account we found form:

http://www.jotform.me/form/83186808214460 collecting credit card information, social security number and driver's licenses.

Credit card information we need to delete in order to reactivate your account.

Forms gathering all other sensitive information should have two mandatory widgets in place.

If your purpose is not to collect these data from the general public, but from your own customers, you just need to comply with two things:

1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

However, we do not allow uploads of any sensitive personal information like: passports, social security cards, driver's licenses, credit/debit cards, statements, bank statements, voided cheques/checks, birth/marriage certificates, visas and photo IDs.

Yes i understand and my purpose is not to collect these data and i also
deleted the form month ago and today i was create the new form and the
login issues
...

if i will create this form so i will use these widgets
