Add a url to the form

I have a question that I need to reveal a url, how? 

Example:

Do you need online registration:  Yes  No

Yes will reveal a url http://helpme.com

No will leave the url hidden

I cannot seem to find a field type that will allow me to add just a label or a line of text. 

Please advise

You can use the TEXT field to add the URL link. Then use Conditions tool to show/hide the TEXT field.
