workflows in the forms?

i'm creating a performance review template that i'd like for employees to do their employee self-assessment then have the clinical director do assessment then have the office manager do their assessment.  How can i have different levels go in and complete the same form?  I'd like to do this for our 350 ees with 5 different positions (ex: dental assistant, dental hygienist, patient coordinator, etc.)

Yes, you can create different sections in the same form with different Headings. You can also use Section Collapse for that so that you can show and hide the whole section based on user response when required.
