    Thanks for a great tool!

    I need some help with a couple of issues, first how can I change the name of my form.  And second how can I creat a button so when I place the code on my page, a button apprears instead of underlined text?

    Hi abelwirz,

    To change the name of the form itself, it's quite simple.

    1. Go to the top left of your form, where the orange bar is.
    2. Click on the title, and change the name.

    About your second question, I viewed your site and all I see is a lot of codes. You might want to fix that first. Anyway, are you trying to embed your form as a lightbox popup? The underlined text only appears when you use the lightbox code.

    You can replace the underlined text with any button/image that you want by doing the following:

    1. Get the lightbox embed code of your form via Setup & Share > Embed Form > Lightbox

    2. In this example:

    <script src="//www.jotform.com/min/g=feedback" type="text/javascript">
    new JotformFeedback({
    formId:'10681505080',
    base:'http://www.jotform.com/',
    windowTitle:'Clone of Hotels',
    background:'#FFA500',
    fontColor:'#FFFFFF',
    type:false,
    height:500,
    width:700
    });
    </script>
    <a id="lightbox-10681505080" style="cursor:pointer;color:blue;text-decoration:underline;">Clone of Hotels</a>

    Replace the text "Clone of Hotels" with your <img> tags so it becomes, for example,

    <script src="//www.jotform.com/min/g=feedback" type="text/javascript">
    new JotformFeedback({
    formId:'10681505080',
    base:'http://www.jotform.com/',
    windowTitle:'Clone of Hotels',
    background:'#FFA500',
    fontColor:'#FFFFFF',
    type:false,
    height:500,
    width:700
    });
    </script>

    <a id="lightbox-10681505080" style="cursor:pointer;color:blue;text-decoration:underline;"><img src="//www.example.com/img/buttonimg.jpg" /></a>
