Which template best suits my needs?

Hi there, I'd like to try out your form but I can't seem to find a good template to start from. I've attached my current order form; I'd love to chat with somebody who could help digitally create something like this. Thanks!

I'm afraid a template won't suffice with a pre-existing form layout like this. What I can recommend is for you to manually add the fields that correspond with the screenshot you shared with us.

For example, if I'm in your shoes, I would start building the form by adding the following fields:

    1. Name field (Name)

    2. Phone field (Phone )

    3. Multiple Choice field (Phgtr)

    4. Short Text Entry field (Agt/Mgr/Refer)

    5. Email field (Email)

    (...and so on)

My advice is to focus on the content first. Make sure that your form will have all the fields you need to collect. Once you're done with the contents, you can focus on the design and layout afterwards. If you need help positioning the fields or moving things around, we have plenty of articles that might help.

For custom CSS, we'll be more than happy to help you here in our support forum. We do offer chat support during US business hours (EST) but it's only available to randomly selected users at a time. If you don't see a chat button while logged in to your account, that means the chat is disabled or not available.

Alternatively, you can also pick a template design that you like then work your way from there. Delete all the fields then manually add the ones that you would need for the form.

Thanks for replying, Jim! I figured that might be the case.

So for from-scratch: My clients have varying numbers of photos they'll have retouched, then specific details for each. How might I set up the option for a client to start with one item (photo to retouch), then they can choose to add more if necessary?
