Please help, signature block does not come through once form is submitted, what have I done wrong?

Hi, I created a form successfully, but in trying a practice submission, my signature block did not come through.  Please advise. Thanks!

I test submitted your form, but I was not able to replicate the issue you mentioned. Perhaps the signature field is blank or empty when the form is submitted?

Could you please check and confirm if my test submission has a signature?
