    Hi Team,

We have a waiver in place now that our parents fill out for kids participating in our studio and at the moment we have made the ability for five fields names to be on it but is it possible to have the ability for the person to add extra ones as required. So essentially fill in five then have a button that says add more and you keep expanding the fields?

Yes it is possible to achieve your requirement by adding following widgets in form which will allow users to duplicate the fields:

    Configurable List

    Infinite List

    Hope this helps.

Do get back to us if you have any questions.

Thank you -- i am thinking the Infinite list might be best so will give it a try

Hi Team, Is there a way to have the data submitted with infinite list option be placed into unique columns so that it can be used

Unfortunately, this cannot be done for our submission page or Google Sheets. But as a workaround, if you export the submissions as an Excel file, MS Excel has a feature that will allow you to split the text into different cells. You can learn about it in detail here: https://support.office.com/en-us/article/Split-text-into-different-columns-with-the-Convert-Text-to-Columns-Wizard-30B14928-5550-41F5-97CA-7A3E9C363ED7
