Link to PDF doesn't look like the form I created

Hi Team, 

I love the layout of your product, but I'm having some issues with the link, which is being generated to access the form. The link is missing fields and formatting. Once I fill out the form I need to convert it to a fully formatted PDF, but the PDF from the submissions page is also missing formating. Please let me know if you can help.

Is your issue related to the PDF report? Your PDF report looks like this.

The only thing that is missing comparing to your online form is the Header image. You can add it inside of the PDF Editor.
