form error

Someone tried to fill out one of my forms online and they weren't able to submit because of an "error." It was the Student Application form in my Field Academy account. Can you help resolve the issue?

I have cloned your Student Application form and was able to successfully fill it out and submit it. Perhaps someone missed a required field while filling out the form?

If you could, please give more information on the kind of error they have received. Did someone else experience a similar issue? Provide screenshots if you have them.

As an additional measure, I have cleared your form's cache which helps resolve some types of unexpected behavior.
