Texts not showing on the PDF copy of submitted forms

Hello,

Please see the attached completed peer Evaluation Form for the 2019 ABA Federal Procurement Institute and the link to the online evaluation jot-form. When I view the completed evaluations, the text describing the sessions that were rated does not show on the completed evaluations.

How do I get the text to show up on these completed evaluations for reference?
