This move-in evaluation for Crestwood Village East lets me publish with the logo, but the PDF will not download.
The move-in evaluation for Crestwood Village West will not let me place the logo in the body of text on the form. It appears as a header, but when I "publish" the form the logo disappears.

It seems PDF form was not generated correctly for some reason. Would it be possible for you to delete the PDF form and create new one?
