How do I change the blurry Pic and Or how do I change the wording ??

Every time I submit an form it looks like the Pic below and I also need to know how to word it in my words? here is the examples the first pic is the lettering down at the bottom where it reads mbr i-go which is the template. the Second Pic is a blurr pic and that is how it shows all the forms when i post them. how do i change that pic to my own logo 

It seems that you are using Facebook messenger to share the form. I have tried scraping the both URLs from the Facebook debugger tool to see if that helps.
