Question about foreign language option

I need to create a patient registration form for a dental clinic that has a Spanish option. Looks like this is no problem, but I have a couple questions:

1. Does the form automatically translate my English written text fields to Spanish or do I have to do that manually?

2. When the patient fills out the form, does it translate their responses (including text field responses) from Spanish to English?

The information sent by the form user will be received in the language that it was originally submitted. So, it would need to be translated using an external tool.
