How to track from where the form was filled out?

does your foms inteegrate with any type of affiliate software? whe i have a hidden field on form to tell me where the form was filled out?

I would suggest you to please take a look at our "Get Referrer" widget and see if that fits into your requirement. This will track the URL from where user comes to fill up form.

If you have embedded same form in multiple webpages and need to find out from which webpage form was filled out, you may like to "Get Form Page URL" widget and see if that fits into your requirement. It will capture the URL of the webpage where the form is embedded.
