Every time I recieve a submission by email and I go to the page where the form is located, it is disabled, so I have to go into the panel and enable it. Is there some setting that is making this happen?

I tested the form in your website http://www.norwoodsoccer.com/content/referee-ar-signals-questionnaire , I was able to successfully submit -- but the form was not disabled afterwards.

I noticed that you had specify a submission limit of 1000 in your form. Removing this limit number will set a No Limit in your form's submissions..
