Why is my alignment messed up?

    I must have hit something but my now my form is out of whack on some of the questions. 

    Some of the options are under the questions and others are to the right. I want them all to the right.

    I have cloned your form and increased the form width to 808 px from advanced designer. Please find the screencast below:
