Trying to create a form that will send to multi congress's address on bills that are being passed , and all I wish for the user to have to do is fill out their info at the bottom. But seem this form  / script I am using don't send to my address at the top , plus it will only take my first_wd40@yahoo.com email address in the info section at bottom., if I use my other email address it says invalid and won't take it.

Can this form do what I am wanting , if not is there one that can?

It should open on the bill page from the below link if not please go to bill page , that is where my form is .

    Thanks David Staggs

If I am not wrong, you want to send the submitted form to multiple addresses. This can be done by adding multiple Email address to notification Email. Please find the screencast below for the same:

If you are looking to send an email alert to your own email address when someone submits your form, normally, you should set up email notifications.

    Setting-Up-Email-Notifications

You can set your email address as the recipient of the email notification that you will be creating. If you need to create more than 1 email notification or send the email notification in your form to multiple recipients you need to have a paid account first. How-to-upgrade-my-account

As for sending email autoresponders to the email address entered in the email field in your form, you can set up email autoresponders in your form.

Not making myself clear .. I am wanting to setup a form somewhat like this page has , doing the same thing..  https://demo.votervoice.net/?vvsrc=%2fcampaigns , click on any of the bills below the header and you can see the form on the right side... I am not getting any emails to my Mailbox from your form and not sure what I am doing wrong .. I was getting the notification and autoresponders that was all I was getting and when I turned them off I stopped getting emails from your form even tho it said it sent BUT I don't need or want the notification of when someone sends it  or the autoresponders replying back to them I just wish to fill out my email in to: and get it  and I am not .. Thank again for replying back

I would like to apologize. I'm still not sure I understand your concern clearly.

I see in your form that you have a field positioned on the left side which has an email address as its default value and the field is also set as "read-only". I can only assume that email address is your email address.

You mentioned that you just want to fill "to:" with your email address and set it as the recipient so you can get emails to your Mailbox. If it is not the email notification set up in your form, may we know where do you want to set your email address as recipient, please?

We may need to get more details from you about your requirements. Also, if possible, you may upload screenshots in your reply if it will help explain your requirements better. How-to-Post-Screenshots-to-Our-Support-Forum

Please go to the link I gave above ... https://demo.votervoice.net/?vvsrc=%2fcampaigns and click on any of the bills that is below the header and you will see the type of form I am after and what I am after.

I am wanting to setup a form to write our congress rep on bills with their Emails address already in , the body already written out and all the user has to do is fill in the bottom of form with their info and click send for the addresses to get it.

I am testing with my email address to make sure it works.. I understand now that I have to upgrade to be able to send to multi address , BUT I want to make sure it works before I go live with it .

The notification and autoresponders works fine , BUT I don't need them ... that was the ONLY email I was getting when I tested . It would notify me that someone (me, in my test) filled it out ... the autoresponders also let me know .

When I turn them off for again I don't need to know when someone fills out my form and send it nor do I need to thank Congress  for reading my letter my sending a auto-responder .

BUT WHEN I turn these off I don't get nothing , no email BUT the form says it was sent , so who got it ?

I have the Email and body lock out so the user can't change it I just wish for the user to be able to put their info and not change anything .. But the field for the user's email ONLY WILL ACCEPT my email of first_wd40@yahoo.com , anything else it says is invalid .

Another way to look at it is ... I am trying to setup a form for users to email multi rep in congress AND ALL the user has to do is fill in their info at the bottom and click send and the people in Congress gets it .

But to do that the Email box for the user at the bottom has to take their email and it has to be able to be sent and received to the address and neither is working

I can change the bottom on the user's Email by taking the box away BUT then I also unlock the body of the letter and when I do that , no telling WHAT some one will write and send .

If you will turn off the email notification or email autoresponder in your form by disabling or deleting, no one will get an email alert when your form is submitted since there were no (active) email notification nor email autoresponder set up in your form.

As for filling up the email field at the bottom of the form embedded your website, I was able to fill it in with a different email address that is not your email address.

Please share with us a screenshot of the email address entered in the email field in your form and is showing an invalid error message.

Also, I can see that you were able to set up the fields in your form where other fields are enabled and other fields are disabled by changing the "Read-Only" state of the field from its properties.
