How can I text a jotform to patient?

    Can we create our very own surveys and text them securely to patients?

    How does the information come back to us? where do we find it? can you export it into an excel spreadsheet

    This is possible also as long as you know your SMS email gateway and the feature is allowed.

    You should ask your mobile service provider for the e-mail access gateway of their system for mobile phone accessing. (Not sure exactly what's the term for it)

    For example is T-Mobile.  Their subscribers can receive SMS messages from e-mail if the sender sends a message to the mobile number, followed by @t-mobile.net 
    Example: 3335551111@t-mobile.net
