Why doesn't the submit button work on an iPad?

Unfortunately, I don't have an iPad to test your form. However, I was using an online emulator to test a cloned version of your form and it seems the submit button is working at my end.

May we know if your form is embedded to a web page?

If yes, can you please share to us the web page URL so that we can check it out?

May we know if there is an error message when you tried to submit the form?
