The integration into a google spreadsheet shows multiple entries, but an email was never generated. Are emails sent immediately at all times? We have had a couple of instances with our Initial Assessment form where a user fills out the form and click's submit but does not receive an email.

If you are referring to this form 81684928441163, I have checked this form and I can see that the email notifications are sent to the users. You can check this under your profile settings >> History. In the last submission, I can see that CSCR Staff Email is brianna.nelson@careersourcecapitalregion.com
