Form Not Found Error

I went to preview my form and am getting a form not found error.  I tried cloning the form and still get the same error.  Says to contact the owner of the form -  I created the form so I don't understand why I am getting this issue.  

I have checked your account and I can see that the form that you are referring to is disabled. That is the reason you are getting this error. You can enable this form from your account and it will work as normal. Please follow the steps in the screenshot below:
