Required fields were submitted empty.

I have 2 required fields that did not show up when the user submitted.  As you can see from the screenshot that Data Destruction? field was answered.  They answered yes to that question.  When conditionally they answer yes then 2 more additional fields should pop up which are required.  They are email and COD.  Somehow the response was submitted, and these fields are not showing.  Please advise.
