When a form is completed them i get an email of the whole form as attached to my email address, but this has stopped working since 16th of this month and i cannot work out why
This is urgent please

I've just created a clone of your last form and set the recipient to my address. It submits without error and sends the email straight away. So my guess is that you might have some problem with your mail box. Have your spam settings changed recently? Please check your junk mail folder. Failing that you should speak with the person who manages your email and ask them to make sure the following addresses are whitelisted:
If that doens't work then please get back to me and we can try something else. Also one other point which might make your forms a little bit better. I noticed that you are using checkboxes for Yes/No answers. That means that your users can select both Yes and No to an individual question. Ideally you should change those checkboxes to radio buttons, so that users can only select one or the other.
