I am using Square to process my forms with payments. Is there a way that you can show me how we can do Cash, Credit or Check and setup the form below to do that?

That's possible. You need to do the following:
1. Add a Single Choice element in the form with your payment options.
**You can also add another Text field with your instructions on how people can pay by Cash or Check.
2. Set up a conditional logic to show the Square field if Credit is choosen:

Add another to show text (assuming that you have added the Text field) when people choose Cash or Check.
This method will hide the fields until the corresponding payment type is chosen in the Single Choice. If Cash or Check is selected, the Credit Card field will remain hidden and will not require people to submit their card details.

Do you have integration to accept ACH for check payments online?
