Counselling Intake Form

    Hi!

    I want to create a counselling intake form for my patients before we start our first session.

    I have a few questions:

    1. Is there a limit as to how many questions can be asked on the form?
    2. Can I make the form interactive? Meaning, if they click on one answer then the next question comes up and if they click on a different answer, then a different question comes up next?
    3. Most important question - can the form generate a report for me after it is submitted? Do I have to go through the form myself to look at the answers or can Jotform summarize the answers for me in paragraphs. My intake form is long and tedious to review after each patient.

    Hi Yuval,

    Field limits depend on your subscription. Here are the limit of fields for each plan:

    • Starter: 100
    • Bronze: 250
    • Silver: 500
    • Gold: 1000

    Please note that basic fields like the "Full Name", "Email" fields and the likes are included in this limit. You can check out our Pricing  page for a quick breakdown of the limits for each plan.
