Show specific box based on user selection.

Are you able to do a drop down form within a form, e.g. if the respondent answers yes, a specific box opens up with choices or form fields that are related to that choice?  Like the "Payment Options" section of the form in the link I included.

 

I'm looking for an alternative solution for my forms because I'm not able to use the complex systems at Luxsci that require coding.  I need a more user friendly option.

Thank you. I'm just playing around with it now. My biggest issue is
formatting.
Why is the line breaking on "An Adult Dependent" even though there is so
much space and the elements above are flowing across the box fine.
Is there a good video to help me learn all the formatting stuff. I used a
template but maybe I should start with a blank form.

With regards to your question on why "An Adult Dependent" is breaking. I checked your form and I don't see the option broken.
