Google suspended my android accident book app - I assume they think I do not have permission to use jotform?

The site is published via appcreator24.com which I believe may add adverts over which I have no control as I use the free service.

I have submitted an appeal to Google.

    Dear Google Play Developer,

Thanks for contacting Google Play. Your appeal has been submitted successfully and will be reviewed by a specialist.

The ticket number for your appeal is referenced in the subject line of this message, and you should receive a response from us within 72 hours.

We ask that you do not send duplicate appeals as this will not reduce response time.

We look forward to working with you,

I am sorry for I was not able to fully understand the issue you reported.

I reviewed your account/username mobileappfactory  and it is active and forms are also active and available as of this checking.

Can you please reiterate what exactly was the problem on your account or forms -- so we can provide you with the proper assistance. I am not sure yet if this is related to your JotForm account and forms.

We will wait for your updated response.
